HDLC Certificate of Appropriateness

I was told by someone in the office that I needed to file for change of use permit. The power was off for a long enough time period that Entergy requires inspection. The unit has 2 meters, of that is for 305 and one for 306. At some point long ago the 2 units were combined. There was an inspection at some point, the part of the unit that is powered by meter for 305 is on. We need to have an general inspection for 306 to send to Entergy so that they will allow power to be turned on. Please help by letting me know if this is not the correct permit so that I may get the power turned on. Thank you
